Your first visit to Shenandoah Dermatology involves a few special steps so that we can get to know you. To understand what to expect, please read through this page. You'll find all the practical information you need, such as a map and directions to our office, practice hours, payment policies and more.

There's also background information about our committed staff and our first visit procedures. You can also save some time by printing out and completing the patient forms in advance of your appointment.

 For your first appointment, we do ask that you bring your medications with you to the office.  If you prefer not to bring your medications in, please bring with you a complete medication list that includes name, dose and frequency of use for each medication (your primary care doctor's office usually provides this list when you have visits with that office).

Minor Policy

All minors MUST be accompanied by a parent or legal guardian at the minor's first appointment. After the minor's initial visit: *Parent or legal guardian MUST be present for any invasive procedure such as liquid nitrogen to treat warts or molluscum, biopsies, injections or blood draws. If the parent or legal guardian is not present, the procedure will not be performed and patients will be asked to schedule another appointment when the legal parent or guardian can be present.

(ACCUTANE) is a high-risk medication used for the treatment of acne. Patient’s parent or legal guardian MUST be available by phone to confirm the patient’s behavior and mental status in order for the patient to obtain their prescription for Accutane. They must also agree to dosing changes and lab test that will need to be performed. If parent or legal guardian is not preset, the patient will be given a lab slip to take to an outside lab. If parent or legal guardian cannot be reached in a timely manner during patients appointment, patient may be asked to reschedule their appointment.

For all minors, the Consent To Treat A Minor form MUST be filled out in its entirety and signed by a parent or legal guardian and will be required to be on file in the minor's chart, to be able to be seen at our practice. If this form has not been completed, the parent or legal guardian will be asked to fill it out and sign it, the day of the minor's appointment. Please also make sure that the parent or legal guardian has provided a password, on the form, as this is very important! The parent or legal guardian will be asked to provide that password, when calling in, to discuss anything regarding the minor, including to verify identity if we need to call during the minor's appointment.

    Please note: If you fill out the online forms, you do not need to print the forms to bring to the office. Our staff will have a copy on file after you submit the online form!

    Prescription Refills

    To request a refill of your medication, please call your pharmacy and ask them to submit a refill request to our office or log in to your patient portal and submit your request. If your prescription is older than two years, you may be required to schedule an appointment with the doctor. Many medications require prior authorization and our office will contact your insurance company to receive approval for the medication you desire. Please allow 72 hours for a normal prescription refill. Do not wait until you are out of medication. Thank you for your cooperation.

    What To Expect

    Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It will also help alleviate any unnecessary anxiety you may be feeling prior to your first appointment. Educate yourself on your symptoms by reviewing the content on this Web site. We ask that ladies present for appointments with  no makeup  as this will allow the physician to provide a thorough examination of your skin. Our office has appointments as early as 7:30 AM and we invite you bring along your makeup and apply it in the office after the appointment if you would like. Also, take some time to review our staff page and familiarize yourself with the doctors. We look forward to your first visit.
